adr is a Claude Code skill that manages Architecture Decision Records — creating, listing, and searching documented architectural choices within a project. The `/adr create` command generates a sequentially numbered file at `docs/decisions/NNNN-<slug>.md` using a structured template with Status, Context, Decision, Consequences sections, and an alternatives comparison table. `/adr list` displays all ADRs with their current statuses (proposed, accepted, deprecated, superseded), while `/adr search` queries titles and content for relevant decisions. The skill integrates with an orchestrator: when task complexity reaches 7 or above, it prompts creating an ADR, and records feed into `vault/projects/{project}/decisions/`.
